Many insurance plans, especially HMO's, require a referral or written authorization from your PCP (primary care physician) for each visit. Your PCP should be able to tell you if you need a referral but ultimately it is your responsibility to know if a referral or written authorization is required before each visit and to obtain one. How soon should I cancel my appointment if I cannot make it?
Patients are required to give at least a 24 hour notice of cancelling any appointment with our practice.
Canceling an appointment or changing an appointment in less than 24 notice will be charged a fee.
Office appointment patients are charged $35.00
Procedure appointment patients are charged $75.00
Exceptions to this will be if the patient can provide documentation of an emergency that has caused them not to be here. Example: death in family, hospital admission, accident etc…
Severe weather conditions will also be permitted to excuse of patients of not being charged a “no show” fee.
A “no show” fee can only be removed and approved by the Billing Specialist and Office Manager.
Yes, please bring your insurance card, co-payment owed, medical records that would pertain to your office appointment, a current medication list or your medications. If you are a new patient to our facility please print out and bring the new patient paperwork along with you to your visit with us to help expedite your office visit. (Andrea – can you put a link to the forms for this?)
